Complaint Letter Templates by Disruption Type
A good complaint letter is under 400 words, cites the regulation, states the amount, and sets a deadline. Here are tested templates for cancellation, delay, denied boarding, baggage, and ADA/ACAA accessibility complaints.
The Common Structure
Every effective complaint letter has five parts: flight identifier, brief facts, regulation citation, amount requested, and deadline. Keep under 400 words for best response rate.

Cancellation Template
Subject: "Refund Request: Flight [number] [date], Booking [ref]"
"On [date], Flight [number] was cancelled. Under 14 CFR 259.5 (DOT final refund rule), I am entitled to a full cash refund to my original payment method. The ticket price was [$amount]. Please process within 7 business days per DOT requirement. If not received by [date + 10 days], I will file a DOT complaint at secure.dot.gov/air-travel-complaint. Reference: [booking ref]."
Delay Template (DOT)
"On [date], Flight [number] was delayed [X hours], arriving at [time] instead of [scheduled time]. Under 14 CFR 259.5, I am entitled to a full cash refund because the arrival delay exceeded 3 hours (domestic) or 6 hours (international). The ticket price was [$amount]. Please process within 7 business days. If not received, I will file a DOT complaint."
Delay Template (EU261 / UK261)
"On [date], Flight [number] arrived [X hours] late at [destination]. Under Regulation (EC) 261/2004, I am entitled to EUR [250/400/600] compensation (route distance: [km]). The delay was not caused by extraordinary circumstances. Please confirm payment within 30 days. If denied, I will escalate to [NEB name] or [AviationADR/CEDR]."
Denied Boarding Template
"On [date], I was involuntarily denied boarding on Flight [number] despite holding a confirmed reservation. Under 14 CFR 250, I am entitled to denied-boarding compensation of [$amount] (based on [delay length] rebooking delay and ticket fare of [$X]). Please process per DOT regulations. Reference: [booking ref]."
Baggage Template
"On [date], Property Irregularity Report [number] was filed at [airport] for [delayed/lost] baggage. Under 14 CFR 254.4 (domestic) or Montreal Convention (international), I am entitled to up to [$3,800 / 1,519 SDR] compensation. Attached is the bag inventory with receipts totaling [$amount]. Please process within 45 days."

ACAA Accessibility Complaint Template
"On [date], Flight [number], I experienced [describe accessibility failure: wheelchair not provided, aisle-chair refused, etc.]. This is a violation of 14 CFR Part 382 (Air Carrier Access Act). I am filing a formal complaint under DOT's ACAA process and requesting [specific remedy: compensation, corrective action, apology]. I will escalate to DOT OCR if not resolved within 30 days."
